If you enrolled or continued coverage in the State of Texas Dental Choice PlanSM
or the DeltaCare®
USA DHMO plan, you will get a welcome letter from Delta Dental in the mail by late August. That letter that includes:
- your member ID number,
- information about how to find an in-network dentist and
- instructions to register for an online account.
You won’t need an ID card. To get services, provide your name, birth date and Delta Dental member ID number when you make an appointment. Any dependents on your plan should provide their name and birth date and your Delta Dental member ID number. (They will not have their own member ID number.)
If you do want a card, you can:
- Register for a Delta Dental online account (after your coverage starts on September 1) to print an ID card.
- Use the Delta Dental mobile app to download one to your smartphone. (Dependents will not have access to the Delta Dental app.)
Please note: Only your name (employee) will be listed on the card. The names of any covered dependents will not be listed on your ID card. Dependents will not receive a card with their names on it.
