If you had non-Medicare health insurance through the Texas Employees Group Benefits Program (GBP) in Calendar Year 2019 (January-December), Form 1095-B is available to you from your health plan provider if you haven't gotten it in the mail already or if you misplaced your copy.
Form 1095-B: Proof of medical coverage
Form 1095-B has information you can use on your individual tax return to verify that you (and any covered dependents) had medical insurance in 2019. If you were in more than one health plan in 2019, you should have more than one Form 1095-B. For example, if you were in Scott and White Health Plan (SWHP) from January through August and switched to HealthSelect of Texas®
in September you’ll get/you should have gotten one Form 1095-B from SWHP and another from Blue Cross and Blue Shield of Texas (BCBSTX).
Remember: You do not need to provide the IRS with a copy of Form 1095-B–your health plan will send them this information electronically. But you may want to keep a copy of Form 1095-B with your tax or insurance records.
Getting your Form 1095-B
The following GBP health plans mailed Form 1095-Bs to members’ addresses on file by January 27.
If you didn’t get it in the mail or misplaced your copy, you can ask for it by calling the health plan toll-free at its number below.
