Choosing optional life, dependent life or disability insurance usually requires this step. Learn what you need to do.

EOI is an application process in which you provide information about your health or that of your dependents. You or your dependents may be denied coverage based on information in the EOI application. It’s standard practice in the insurance industry to require EOI for certain types of insurance, although ERS waives the requirement for some coverage if you enroll during your first 31 days of employment.

Who conducts the EOI process

The insurer or the insurer’s underwriter will oversee the EOI process. Securian Financial Group, Inc. oversees EOI for Optional and Dependent Term Life Insurance. Guardian Life Insurance is the underwriter for TIPP disability insurance.

How to submit your EOI

Initiate the EOI process online after you request to enroll in Optional or Dependent Term Life Insurance and/or TIPP disability insurance. You can choose whether you want the EOI underwriter to communicate with you by email or mail. Then:

  • The EOI underwriter will provide instructions for submitting your EOI application.
  • You must answer all questions on the EOI application truthfully and completely. Missing information can delay the process.
  • If needed, the EOI underwriter will request additional information to make a decision on your application.
